After the latest release, on some of our sites, we were getting a redirect loop the first time someone logged in due to the redirect to the upgrade notes page within the WP backend.
I am also getting this redirect loop, on just one least edited site within a network. The loop occurs only on the dashboard page. I am guessing it has something to do with your update splash screen. I'm also getting the same issue on a multisite network running WP 4. Only a small percentage of sites are affected. Could you add a way to override or remove the splash screen?

Saturday I waas doing some correcting of some duplicate titles, including nofollowing tags, etc. Generally the Wordpress core systems work great, its other modules that tend to cause issues. I don't see a specific problem with the htacess, did you install any other redirecting scripts like canonical redirects? It sounds like this maybe a somewhat common issue and you may find quicker answers to this sort of technical issue directly on the Wordpress forums.
